Your child has been invited to participate in a live performance event at The Sundown. Please read the information below and sign to indicate your consent.
Parental/Guardian Responsibility
I understand that this event will take place at a licensed music venue. By signing this form, I acknowledge and agree that:
I am responsible for the supervision, safety, and behaviour of my child at all times during the event.
The event organiser and venue staff are not responsible for the direct supervision of my child.
I will remain present at the venue for the duration of the event unless otherwise agreed in writing.
2. Photography and Video Content
I understand that photos and videos may be taken during the event, including recordings of my child’s performance. By signing this form, I agree that:
The event organiser may capture photos and video footage of my child during the event.
This content may be used for promotional or educational purposes (e.g. social media, website, direct sharing with myself).
No photos or videos featuring my child will be shared publicly by the organiser without additional specific consent being obtained from me (e.g. if the child is not happy with their performance, and does not want it shared, it will not be).
